Her breakthrough role came in 1944, when she played the part of Ida, the cockney maid, in the original production of See How They Run at the Q Theatre. The play's success led to a West End transfer to the Comedy Theatre in January 1945.

In 1961, Hickson played the housekeeper in the film Murder, She Said, based on Agatha Christie's novel 4.50 From Paddington. Although she didn't play Miss Marple in this film, Christie wrote to her, "I hope one day you will play my dear Miss Marple."
That wish was fulfilled when Hickson took on the iconic role of Miss Marple in the BBC television series Miss Marple, which aired from 1984 to 1992. Her performance as the gentle, yet keen-witted detective earned her widespread acclaim and a loyal following.
For her outstanding contribution to British television, Hickson was appointed an Officer of the Order of the British Empire (OBE) in 1987.
