He later moved to Serie C1 side Massese, where he made 22 appearances before joining his hometown club Triestina. In the 1994 summer transfer window, Milanese was signed by Serie A side Cremonese, marking his top-flight debut.
The Italian defender then went on to play for Torino and Napoli, again at the Serie A level. In the summer of 1997, he was signed by Parma, where he spent half a season before being loaned to Internazionale. Milanese played his first match for Inter on January 25, 1998, as a substitute for Youri Djorkaeff in a 1-1 draw against Empoli.
