In 2003, he compiled a record of 17-4 with a 2.06 ERA in 24 starts for the Single-A Lake County Captains, earning him the Most Outstanding Player of the South Atlantic League award, as well as All-Star honors and the Indians' 2003 Minor League Player of the Year award.
Hernndez made his MLB debut on April 15, 2006, starting against the Detroit Tigers at Comerica Park. He pitched six innings, giving up one earned run and striking out four batters. Despite his promising start, Hernndez's career was marred by controversy when his true identity was revealed in 2012. He had been playing under the alias "Fausto Carmona" and had claimed to be three years younger than his actual age to obtain a visa to pitch in the United States.
