  1. Preheat the oven to 300°F (150°C).
  2. In a medium saucepan, whisk together the heavy cream, granulated sugar, whole milk, and instant coffee powder. Heat over medium heat, stirring occasionally, until the sugar has dissolved and the mixture is hot but not boiling.
  3. In a small bowl, whisk together the egg yolks and melted butter. Gradually add the warm cream mixture to the egg yolks, whisking constantly to prevent the eggs from scrambling.
  4. Strain the mixture into a clean bowl and whisk in the vanilla extract. Pour the mixture into 6 ramekins or small baking dishes.
  5. Place the ramekins in a large baking dish and add hot water to come halfway up the sides of the ramekins. Bake for 25-30 minutes, or until the edges are set and the centers are still slightly jiggly.
  6. Remove the ramekins from the baking dish and let them cool to room temperature. Cover and refrigerate for at least 2 hours or overnight.
  7. Just before serving, sprinkle a thin layer of confectioners' sugar over the top of each ramekin. Caramelize the sugar with a torch, if desired.
