  2. In a medium saucepan, combine the heavy cream, granulated sugar, and salt. Heat over medium heat, stirring occasionally, until the sugar has dissolved and the mixture is hot but not boiling.
  3. In a small bowl, whisk together the egg yolks and vanilla extract. Gradually pour the warm cream mixture into the egg yolks, whisking constantly to prevent the eggs from scrambling.
  4. Return the mixture to the saucepan and cook over low heat, stirring constantly, until the mixture thickens and coats the back of a spoon, about 10-12 minutes.
  5. Strain the mixture into a clean bowl and stir in the cooked tapioca pearls. Let the mixture cool to room temperature, then cover and refrigerate for at least 2 hours or overnight.
  6. Preheat the oven to 300°F (150°C). Pour the chilled mixture into ramekins and place them in a large baking dish. Add hot water to come halfway up the sides of the ramekins and bake for 25-30 minutes, or until set.
  7. Remove the ramekins from the baking dish and let them cool to room temperature. Cover and refrigerate for at least 2 hours or overnight. Just before serving, sprinkle a thin layer of granulated sugar over the top of each ramekin and caramelize with a torch.
