Her father, Gran von Otter, was a Swedish diplomat who served in Berlin during World War II. Von Otter's early exposure to music led her to pursue formal training at the Guildhall School of Music and Drama in London, where she studied under the guidance of Vera Rzsa.
In 1982, von Otter won second prize at the ARD International Music Competition, marking the beginning of her successful career. She became an ensemble member of the Basel Opera from 1983 to 1985, making her professional operatic debut as Alcina in Haydn's Orlando paladino. Her subsequent performances at the Royal Opera House, Covent Garden (1985), La Scala (1987), and the Metropolitan Opera (1988) solidified her position as a leading mezzo-soprano.
