In 1915, King Constantine I appointed Gounaris as Prime Minister, following Venizelos' first resignation. He escaped to Sardinia, Italy, in 1918 and eventually returned to Greece in 1920, where he played a pivotal role in the November elections as the leader of the United Opposition.
After Venizelos' defeat, Gounaris controlled a majority of the parliament and became the driving force behind the royalist governments. In March 1921, he assumed the office of Prime Minister, marking the beginning of his second premiership.
